29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: On or about February 16, 2017, and times prior thereto, for employees stick welding a 10 inch steel gas pipe in a trench greater than 5 feet in depth, an inspection of the possible cave-in hazards was not conducted by a competent person.
