29 CFR 1910.147(c)(4)(ii)(B): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a. Haviland Plastic Products Inc. located in Haviland Ohio: On or about November 16, 2016, the employer did not ensure the machine specific lockout procedures for machine #20 include instructions on dissipating residual heat from the die and sheet rollers prior to performing servicing or maintenance on the equipment. Employees were exposed to hot surfaces from the die and sheet rollers, on machine #20 located in the production area, while removing the die from the equipment.
