29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On or about August 4, 2016, located at the southwestern portion of the 3rd floor at the former Columbus Municipal Light Plant, there was an uncovered 24 inch i.d. floor hole where employees were removing scrap material, thereby exposing employees to trip/fall hazards. b. On or about August 4, 2016, located at the eastern generator on the 3rd floor at the former Columbus Municipal Light Plant, there were two uncovered 7-7/8 inch floor holes and a 9 x 20-3/4 inch floor hole where employees were removing scrap material, thereby exposing the employees to trip/fall hazards. c. On or about August 4, 2016, located at the southwestern portion of the 3rd floor at the former Columbus Municipal Light Plant, there were four uncovered 12 inch i.d. floor holes where employees were removing scrap material, thereby exposing the employees to trip/fall hazards. d. On or about August 4, 2016, located at the southwestern portion of the 3rd floor at the former Columbus Municipal Light Plant, there was an uncovered 25-1/2 inch diameter floor hole where employee were removing scrap material, thereby exposing the employees to trip/fall hazards. e. On or about August 4, 2016, located on the 6th floor at the former Columbus Municipal Light Plant, there were three 17-1/2 inch diameter floor holes where employees cut and removed scrap material, thereby exposing the employees to a trip/fall hazard. f. On or about August 4, 2016, located on the southeastern stairway landing between the 6th and 7th floors, there was a 28 inch diameter uncovered floor hole in the middle of the landing where employees walked with cutting equipment and removed scrap materials, thereby exposing the employees to a trip/fall hazard. g. On or about August 4, 2016, located at the generator on the eastern portion on the 3rd floor of the building at the former Columbus Municipal Light Plant, there was a 6 inch by 12 inch floor hole and a three foot by four foot floor hole where employees removed scrap material with the use of cutting torches, exposing the employees to a trip/fall hazard. h. On or about August 4, 2016, located near the northern 3rd floor stairs going down to the second floor at the former Columbus Municipal Light Plant, there was a 10 inch by 10-1/2 inch floor hole where employees gained access to the stairs and carried scrap material, thereby exposing the employees to a trip/fall hazard. i. On or about August 4, 2016, located at the generator on the northwestern portion of the building at the former Columbus Municipal Light Plant, there were two 7-7/8 inch floor holes in the floor, thereby exposing the employees to a trip/fall hazard.
