29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Instance 1: On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 6' shock absorbing lanyard and attached to another web sling 6' lanyard increasing the free fall distance more than 6 feet. Instance 2: On or about January 7, 2016, at the above addressed job site, an employee was laying cement block units on a 3rd floor slab while being attached to a 9' self-retractable lanyard and attached to a 6' cable that was attached to an anchor embedded in the concrete slab increasing the free fall distance more than 6 feet.
