29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times. On 13 January 2016, and at times prior thereto, smoke detectors located in the common areas of the facility, such as the break rooms and offices, were inoperable due to no battery, battery installed backward, or dead battery, exposing employees to fire hazards and smoke inhalation. A) HSE Office: The Kidde Photoelectric Smoke Alarm, Single Station, Model P9050 located on the east wall of the HSE (Health, Safety & Environmental) office was missing a battery at the time of the inspection making it inoperable. B) Large Break Room: The Kidde Photoelectric Smoke Alarm, Single Station, Model P9050 located on the north wall of the break room had a dead battery at the time of the inspection making it inoperable. C) Small Break Room: The Kidde Photoelectric Smoke Alarm, Single Station, Model P9050 located on the east wall of the break room had a battery installed backwards at the time of the inspection making it inoperable.
