29 CFR 1910.29(a)(3)(v): The work level platform of scaffolds (towers) were not of wood, aluminum, or plywood planking, steel or expanded metal, for the full width of the scaffold, except for necessary openings and work platforms were not secured in place. On or about 13 January 2016 and at times prior thereto, two workers were performing welding and cutting operations from a mobile tubular welded scaffold which was not fully planked and the planks were not secured, exposing the workers to the hazard of falling approximately 8 feet, 6 inches to the floor below.
