29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: Throughout the facility: On or about October 1, 2015 through October 28, 2015 (when the OSHA form 300 was received), the employer did not record the following workplace injuries or illnesses within seven calendar days on the OSHA form 300 or its equivalent for the calendar year 2015: a) AFE: On 10/14/15 an employee experienced low back pain when performing box replenishment on 10/13/15. The employee was placed on job transfer or work restriction. b) Pick area: On 10/12/15 an employee felt pain in their lower back after lifting a part from a bin. The employee was placed on job transfer or work restriction. c) Stow: On 10/1/15 an employee experienced pain to their right forearm and hand from repetitive work operations lifting products. The employee was placed on job transfer or work restriction. d) Pack Singles: On 10/2/15 an employee was moving product and experienced neck and shoulder pain. The employee was placed on job transfer or work restriction, prescribed physical therapy, prescribed medications and the injury involved days away from work. e) Pack Singles: On 10/16/15 an employee was moving items and developed left arm pain. The employee was placed on job transfer or work restriction, prescribed medications and injury involved days away from work. f) Stow: On 10/2/15 an employee reached out to push a box out of the way and jammed their right pinkie finger causing pain. The employee was placed on job transfer or work restriction. g) Pick area: On 10/8/15 an employee reported right knee pain from repetitive bending for product. The employee was placed on job transfer or work restriction. h) Ship support: On 10/12/15 an employee dropped a box on their right foot resulting in a fracture to a metatarsal bone. The employee was placed on job transfer or work restriction, prescribed medications and injury involved days away from work. i) Stow: On 10/11/15 an employee reported hand pain from repetitive moving of parts. The employee was placed on job transfer or work restriction and the injury involved days away from work. j) AFE: On 10/1/15 an employee tripped on boxes experiencing pain in their left wrist. The employee was placed on job transfer or work restriction , prescribed physical therapy and the injury involved days away from work. k) Stow: On 10/13/15 an employee experienced right shoulder pain after hearing a pop when lifting a box. The employee was placed on job transfer or work restriction and prescribed physical therapy. l) AFE: On 10/2/15 an employee reported pain and swelling to their right foot from a scanner landing on it during their work shift on 10/1/15. The employee was placed on job transfer or work restriction. m) AFE: On 10/3/15 an employee reported increased pain to both knees during prolonged rebinning work. The employee was prescribed medications and the injury involved days away from work . n) AFE: On 10/9/15 an employee experienced back pain during breakdown of boxes. The employee was placed on job transfer or work restriction and the injury involved days away from work. o) Pick area: On 10/16/15 an employee jammed their left middle finger reaching for a box experiencing pain. The employee was placed on job transfer or work restriction.
