29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or about June 25, 2015, the employer permitted employee(s) to work in close proximity of an energized Motor Control Center 277/480 volt circuit which exposed the employee(s) to shock, electrocution and/or injuries from arc flash hazards.
