29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning. (a) On or about February 3, 2015 at worksite 5695 Davinci Lane, Carthage Missouri.- Employer did not develop, document and utilize alternative measures when performing sanitation on the Vemeg Hopper. Employees were exposed to serious amputation, crushing and caught in hazards. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
