29 CFR 1915.81(a)(4): The employer did not maintain easy and open access to each fire-alarm box, fire-call station, fire-fighting equipment, and each exit, including ladders, staircases, scaffolds, and gangways. a) West end of Building 256: Access to the exit door was blocked by four electrical panel covers which were set directly in front of the exit door and they were leaning into it. b) East end of Building 256: Access to the exit door was blocked by a wooden crate full of 4 inch PVC pipe and metal frames set directly in front of the door.
