29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: a) Center of Pipe Shop: A 120 volt extension cord used to provide power to portable fan had a 1.5 inch gouge in the outer insulation which exposed the wires underneath including some areas where bare wire was showing, it was covered with black tape and was still used in that condition.
