Text For Citation: 02 Item/Group: 005 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to serious injury or death as a result of Class 1 Division 1 explosives, operations being conducted from remote locations. Operations did not meet the required distances to prevent injury caused by blast overpressures of 2.3 psi: a) Glaze Barrel #2; b) Glaze Barrel #3; c) Blender Packer Building; d) Glaze and Screen Building. Among other methods one feasible and acceptable method to correct the hazard is to move the remote control rooms to the distances reflected by Table 6-1 Column B of the DOD 4145-26M Chapter 4 Section B-(13)(d)(8)(e) and accepted Industry Practices. Violation occurred on 6/3/89.
