Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, outside, west side of the building, on or about February 10, 2016 and times prior thereto: A sheet of plywood that was covering a pit had a 14-inch by 18-inch opening in the plywood.
