29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, were not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening were not constantly attended by someone or were not protected on all exposed sides by removable standard railings: A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, basement, on or about February 10, 2016: Employees walk in proximity to an unguarded 28-inch deep pit containing a molasses pump to gain access to areas including, but not limited to, a restroom and the bird seed mixer. This ongoing violation was most recently observed on February 10, 2016. To abate this ongoing violation, the employer must erect a guardrail system around the pit. Mattocks Five, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(5), which was contained in OSHA inspection number 924399, citation number 1, item number 8-b and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 73 Canal Street, Albion, PA 16401.
