29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: On or about 6/21/2019, at the Cincinatti shears, employees were exposed to amputation and struck by injuries while conducting maintenance activities where the employer failed to require each worker involved to apply their own locks.
