29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose, and in completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, first floor, on or about February 10, 2016: Employees are exposed to a start / stop button control box that is not equipped with a cover plate. This ongoing violation was most recently observed on February 10, 2016. To abate this ongoing violation, the employer must securely affix a cover plate onto the start / stop control button. B.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, first floor, on or about February 10, 2016: Employees are exposed to an electrical receptacle box that is not equipped with a cover plate. The cover plate was not securely fastened onto the receptacle box and was dangling to the side. The condition exposed live conductors within the box. This ongoing violation was most recently observed on February 10, 2016. To abate this ongoing violation, the employer must securely affix a cover plate onto the electrical receptacle box. Mattocks Five, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 924399, citation number 1, item number 10-c and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 73 Canal Street, Albion, PA 16401.
