29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016: Employees that are provided with, and permitted to use, portable dry chemical fire extinguishers are not trained at least annually on the proper selection and use of the portable fire extinguishers. Portable dry chemical fire extinguishers are distributed in the plant work areas for employee use. This ongoing violation was most recently observed on April 27, 2016. To abate this ongoing violation, the employer must train employees annually on the proper selection and use of the portable fire extinguishers. B.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016: Employees that are provided with, and permitted to use, portable dry chemical fire extinguishers are not trained upon initial employment on the proper selection and use of the portable fire extinguishers. Mattocks Five, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(1), which was contained in OSHA inspection number 924399, citation number 1, item number 4-c and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 73 Canal Street, Albion, PA 16401.
