29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): (a) 3008 Penns Valley Road, Centre Hall, PA; Engine Room - On or about October 11, 2023, the device(s) used to lockout and tagout the 480VAC disconnect in the electrical panel behind the rolling wall did not identify the employee who applied the lock and tag. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
