29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: In the following locations, unused openings in electrical boxes were not effectively closed. Employees were exposed to electrical hazards: 1. Third floor working area, an outlet box marked "PNL L# K20 CKT #2-4" was missing a knock out. The outlet box was energized. 2. Carpenter Shop: A junction box that was located under and connected to the "Mercury Lamp Ballast" was missing a knock out and was energized.
