29 CFR 1926.1053(b)(3): Ladders were loaded beyond their manufacturer's rated capacity: (a) First Assembly of God Church: On or about November 17, 2017, and times prior, a 24ft. aluminum Werner extension ladder was used to gain access to the roof and was missing the identification labels that list the type of ladder and the maximum intended load carrying capacities of the ladder.
