29 CFR 1910.219(c)(4)(i): Projecting shaft ends. Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a) On or about January 28, 2025, at the establishment located at 2441 East Glendale Avenue, Appleton, Wisconsin, the employer did not ensure the projecting shaft end of the anilox roll on the Vireo 3 Wolverine Print Unit that was 2-1/8 inches in diameter and 4 inches long was guarded. b) On or about January 28, 2025, at the establishment located at 2441 East Glendale Avenue, Appleton, Wisconsin; the employer did not ensure the projecting shaft end of the Print Roll on the Vireo 3 Wolverine Print Unit that was 1-7/8 inches in diameter and 3-1/2 inches long was guarded.
