29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) The employer does not ensure that their licensed explosive demolition contractor SCM instructs Adamo's employees in the recognition and avoidance of unsafe conditions or other hazards while conducting explosive preparation such as structural columns being cut to weaken and direct the collapse of columns in a predetermined direction and conducting demolition processes such as working from ground level to cut structural columns exposing employees and those working around them to struck-by and crushing hazards. The most recent incident of such failure occurred on December 9, 2020, located at 14869 U.S. Highway 52, Manchester, OH 45144. b) Adamo Demolition Company does not ensure that their employees are instructed in the recognition and avoidance of unsafe conditions or other hazards while conducting explosive preparation such as structural columns being cut with relief hinge cuts, flat cuts, triangle pie cuts, lateral shape charge cuts, and top and bottom cuts on structural columns to weaken and direct the collapse of columns in a predetermined direction and, conducting demolition processes such as working from ground level and inside man lifts cutting structural columns exposing employees and those working around them to struck-by and crushing hazards. The most recent incident of such failure occurred on December 9, 2020, located at 14869 U.S. Highway 52, Manchester, OH 45144. The Adamo Industrial Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 C.F.R. 1926.21(b)(2), which was contained in OSHA Inspection Number 1110029, citation number 1, item number 3 and was affirmed as a final order on 09/25/2017, with respect to a workplace located at 1501 Sparling Road Site Waterford, OH, 45786.
