29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Grangaard Construction, Inc. at the Highway 212 Bridge, Watertown, SD 57201: On or about and at times prior to October 19, 2019 the exposing employer failed to report a hospitalization as a result of a fall from approximately 20 feet within the third cofferdam.
