29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: On or about 3/27/2019, at Twins Twins LLC, located at 720 S Main Street, Labelle, FL 33935. The employer did not record a lost-time work-related injury that took place on 3/15/2019, when an employee suffered a partial amputation of left index finger while working with a tortilla manufacturing machine.
