29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: 9000 River Rd, Delair, NJ, 08110: As of 4/12/2017 the employer had not recorded the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2017. a) Anodizing: On 2/3/2017 an employee was inside of the caustic tank removing sludge and received chemical burns on his left leg and ankle. The employee received medical treatment with the use of a prescription antibiotic cream on 2/7/2017. b) Anodizing: On 2/3/2017 an employee was inside of the caustic tank removing sludge and received chemical burns on his hands and scalp. The employee received a prescription for an antibiotic on 2/7/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
