29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016: Employees are performing servicing and maintenance on equipment including, but not limited to, shaker bed/corn cracker. Lockout / tagout is not being employed when performing the service and maintenance task. This ongoing violation was most recently observed on April 27, 2016. To abate this ongoing violation, the employer must enforce the use of lockout / tagout when performing servicing and maintenance activities. B.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016: Employees are performing servicing and maintenance on equipment including, but not limited to, the bird seed mixer. Lockout / tagout is not being employed when performing the service and maintenance task. This ongoing violation was most recently observed on April 27, 2016. To abate this ongoing violation, the employer must enforce the use of lockout / tagout when performing servicing and maintenance activities. Mattocks Five, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 924399, citation number 1, item number 9-a and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 73 Canal Street, Albion, PA 16401.
