29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about June 24, 2015, at Southern Fiber of Miami LLC, Miami Gardens, FL, the employer did not record a lost-time work-related injury that took place on May 22, 2015, when an employee suffered the degloving of his right hand while cleaning polyester fiber from a machine (i.e. a picker). Abatement Certification and Documentation is required.
