29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Located at Aluminum Shapes LLC 9000 River Rd, Delair, NJ: On or about 3/24/15, an employee received an injury to his leg resulting in a hospitalization. The employer did not report the hospitalization of an employee to OSHA within 24 hours. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
