Maintenance Shop: On, or about, February 3, 2016, a microwave oven and a toaster oven were energized through an extension cord. B. Coater 4: On, or about, February 3, 2016, a knockout box designed for permanent wiring was used in an extension cord assembly to energize a Graco displacement pump.
