29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Blasting Booth Area: On or about 4/18/2022, extension cords were observed to be split and spliced into the RadioShack 3 amp Regulated Power Supply used to power the blasting hose deadman switch and the blasting hose lights. b) Blasting Booth Area: On or about 4/18/2022, a connection between the tail of the West Tech deadman switch and the cord for the blasting hose was split and exposing internal wiring. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
