29 CFR 1915.505(f)(2)(i): The fire response equipment was not kept in a state of readiness: (a) At the entrance to the M/v Matanuska at Vigor Marine, LLC shipyard at 5555 N Channel Ave, Portland, Oregon: On or about 2/2/2018 and at times prior thereto, employees were exposed to smoke inhalation hazards and burns in that the employer did not maintain the fire alarm horn in working condition.
