29 CFR 1910.178(q)(7): Industrial trucks were placed in service after the examination revealed condition(s) adversely affecting the safety of the vehicle. Reported defects were not corrected. (a) In the warehouse at the facility at 50 Oxford Ave, on 8/18/2016, and at times earlier, the Powered Industrial Truck identified as FCG 15 was placed in service after the examination revealed defective head and tail lights, defective dashboard instruments, hour meter, and hydraulic leaks adversely affecting the safety of the vehicle. Reported defects were not corrected. (b) In the warehouse at the facility at 50 Oxford Ave, on 8/18/2016, and at times earlier, the Powered Industrial Truck identified as M0038 was placed in service after the examination revealed defective back up lights and back up sound device, adversely affecting the safety of the vehicle. Reported defects were not corrected.
