29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose. The employer does not ensure fittings were provided with covers approved for their purpose. This violation was observed on or about October 18, 2017 at worksite at 19 Bailey Street, Plains Township, PA when employees were working at: 1. The North side of International Paper Box Machine Company Swifty Right Angle Gluer #2 Model FZ (Serial No. 397) with an uncovered conduit outlet body that was two (2) foot, four (4) inches above floor level 2. The South side of International Paper Box Machine Company Speed Queen Gluer #3 Model KH (Serial No. 102) with an uncovered conduit outlet body that was two (2) foot, two (2) inches above floor level, as well as on two prior occasions as described bellow. To abate this violation, the employer must ensure that fittings are provided with approved covers. Midvale Paper Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 1066679, citation number 3, item number 6 and was affirmed as a final order on June 17, 2017, with respect to a workplace located at 19 Bailey Street, Plains Township, PA. Midvale Paper Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 954509, citation number 1, item number 4b and was affirmed as a final order on February 20, 2014, with respect to a workplace located at 19 Bailey Street, Plains Township, PA.
