29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Shop Area: On or about March 9, 2016, and times prior, a 1 1/2 inch opening was measured between the abrasive wheel and safety guard on the Blackhawke bench grinder, Model number 75-8, used to grind burrs off metal and to grind rough edges off bracket(s). Wayne Lumber and Mulch, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, Abrasive Wheel Machinery, which was contained in OSHA inspection number 961178, citation number 1, item number 10b and was affirmed as a final order on September 25, 2014, with respect to a workplace located in Wayne, WV.
