29 CFR 1910.303(b)(2): Electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: A. Wax Room, Soluble Press Area: On, or about, September 14, 2015 a portable Lasko Cyclone fan was not designed for use in an industrial location in that it was not provided with a hard service or junior hard service power cord that included an equipment grounding conductor. B. Wax Room, CMM Work Area: On, or about, September 14, 2015, relocatable power taps were connected in series to energize computer equipment used with the Brown & Sharpe CMM machine. C. Welding, Weld Booth 2: On, or about, September 21, 2015, an extension cord assembly that included a knockout box designed for permanent mounting was used to energize a water chiller provided for a Miller TIG welding unit. D. Finishing, OAP Booth, C-36: On, or about, September 21, 2015, an extension cord assembly was used to energize a relocatable power tap used on a mobile computer cart. The extension cord assembly included a quad knockout box designed for permanent mounting. Also, a second extension cord was plugged into quad receptacle and was used to energize a second relocatable power tap.
