29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) In the gas room, production area, Lines 2-5, at the facility at 50 Oxford Ave, on 6/7/2016, and at times earlier, employees serviced the Gas Propellant Injection Machine by performing tasks such as, but not limited to adjusting sensors, cleaning gas heads, changing gas heads, and removing ruptured cans and associated debris without having received required training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.
