29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On February 20, 2024, carpenter's shop: The electrical cord connecting the on/off button to the Delta Unisaw table saw was damaged and cut, exposing live conductors. Employees were exposed to electrical shock and fire hazards.
