29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed. The employer does not provide enclosures to protect employees at the #2 Zerand Printing, Stripping, and Die Cutting machine with unenclosed sprocket wheels and chains that are seven feet or less from the floor level. This violation was most recently observed on or about October 18, 2017 at a worksite at 19 Bailey Street, Plains Township, PA when employees worked and were exposed to at least four (4) unenclosed sprocket wheels and a chain that ranged between less than one (1) inch to twenty (20) inches above the floor level on the #2 Zerand Printing, Stripping, and Die Cutting machine as well as on one prior occasion, as described below. To abate this violation, the employer must ensure that each sprocket wheel and chain seven (7) feet or less above floors are enclosed to protect employees working at the #2 Zerand Printing, Stripping, and Die Cutting machine. Midvale Paper Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(f)(3), which was contained in OSHA inspection number 994212, citation number 1, item number 3c and was affirmed as a final order on June 17, 2017, with respect to a workplace located at 19 Bailey Street, Plains Township, PA.
