29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: On or about September 1, 2016, employees were exposed to unprotected electrical wiring at the #2222 Denison 6,000 pound trim press. At the bottom of the main disconnect switch for the press the flexible conduit had a rigid elbow fitting that was missing the protective cover plate.
