29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). The employer does not provide guarding to protect employees in the machine area from pulleys with parts seven feet or less from the floor in accordance with the requirements specified in 29 CFR 1910.219 (m) and (o). This violation was most recently observed on or about October 18, 2017 at a worksite at 19 Bailey Street, Plains Township, PA, at the: #2 Zerand Printing, Stripping, and Die Cutting machine, employees were exposed to at least four (4) unguarded transmission pulleys seven feet or less from the floor level that ranged between twenty-six (26) inches and forty-two (42) inches above the floor level as well as on one prior occasion, as described below. To abate this violation, the employer must ensure that each pulley with parts seven feet from the floor is guarded to protect employees working at the #2 Zerand Printing, Stripping, and Die Cutting machine. Midvale Paper Box Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(d)(1), which was contained in OSHA inspection number 954509, citation number 1, item number 3a and was affirmed as a final order on February 20, 2014, with respect to a workplace located at 19 Bailey Street, Plains Township, PA.
