29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) In the gas room, production area, Lines 2-4, at the facility at 50 Oxford Ave, on 9/21/2016, and at times earlier, procedures were not developed for the control of potentially hazardous energy for the propellant charging machines on lines 2-4 when employees were engaged in, but not limited to, cleaning gas heads, and changing of gas heads.
