29 CFR 1926.602(c)(1)(viii)(A):The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: On or about April 4, 2024 the employer BMB Construction LLC at jobsite located at 41 Gone Fishing Ln. Lakeside/Marblehead OH 43440, did not ensure the powered industrial truck Gradall 534D9-45 rough terrain forklift (S/N: 0744030) was equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel. One employee was exposed to falls of 10 feet and 9 inches (from the elevated scaffold pallet) or greater to the ground below while working from an unsecured wooden pallet to install construction framing material. BMB Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard CFR 1926.602(c)(1)(viii)(A) which was contained in OSHA inspection number 1483553, citation number one, item number one-C and was affirmed as a final order on September 10, 2020 with respect to a workplace located at 3340 Liberty Avenue, Pittsburgh, PA 15201.
