29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: On or about December 19, 2018, and at times prior thereto, and at times thereafter, the employer allowed employees to operated forklifts without ensuring that examinations were conducted prior to placing the forklifts into service; and On or about January 10, 2019, and at times prior thereto, the employer allowed employees to continue to operate Toyota forklifts without examining them prior to them being placed in service. Items that needed to be repaired are listed below; these are items that CSHO saw: a. installed back up mirrors were broken; b. back up alarm and associated flashing lights not operable when used; c. installed headlights were broken; d. unauthorized electrical devices used on forklift as a switch; e. protective covers missing on dash exposing electrical parts; and f. foot pedals missing rubber non-slip covers. Southern Tire Mart was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 1318784, citation number 1, item number 9B and was affirmed as a final order on November 1, 2018, with a final abatement date of December 4, 2018, with respect to a workplace located at 75 Pinyon Road, Covington, GA 30016.
