29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 5/24/2018, at the sentinel window department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. The electrical cord for the pedal of a hydraulic punch press (i.e. Die #104) lacked strain relief, exposing an employee to an electrical hazard.
