29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Planer Mill (Stacker); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in and around the Stacker, in that the employer did not provide employees who clean and clear jams with lockout devices to secure the hazardous energies present on the equipment. b) Planer Mill (Strapper); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in and around the Strapper, in that the employer did not provide employees who clean and clear jams with lockout devices to secure the hazardous energies present on the equipment. c) Planer Mill (Tilt Hoist and Planer infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in and around the Tilt Hoist and Planer infeed conveyors, in that the employer did not provide employees who clean with lockout devices to secure the hazardous energies present on the equipment. d) Sawmill (Log Deck and Bucksaw infeed); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in and around the Log Deck and Bucksaw infeed conveyors, in that the employer did not provide employees who clean and clear jams with lockout devices to secure the hazardous energies present on the equipment. e) Sawmill (Unscrambler and Stacker); On or about May 31, 2017 and at times prior thereto, the employer exposed employees to caught-in and struck-by hazards in and around the Unscrambler and Stacker, in that the employer did not provide employees who clear jams with lockout devices to secure the hazardous energies present on the equipment. Dudley Lumber Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(5)(i), which was contained in OSHA inspection number 934646, citation number 1, item number 4 and was affirmed as a final order on March 4, 2014, with respect to a workplace located at 8575 Hwy 280 East, Salem, AL 36874.
