29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Welding station: A 4 inch x4 inch metal receptacle box was not mounted to a wall or installed in accordance to the manufacturer instructions and was used as a power strip to power a phone and a hand held grinder, observed on or about 7/23/2015. b) Shipping department: Two power strips were used to power equipment such a Ryobi cutter and hand held drills, on or about 05/27/2015. United Hospital Supply was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 314581422, Citation Number 1, item Number 16 and was affirmed as a final order on 11/24/2010, with respect to a workplace located at 4422 Route 130 South, Burlington NJ 08016.
