29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) 9000 River Rd Delair, NJ- Building 1, Fabrication Area : Employees operating a metal cutoff saw (machine 888) to cut materials such as but not limited to aluminum were exposed to amputation hazards created by unguarded saw blades, occurred on or about 5/13/15. b) 9000 River Rd Delair, NJ- Building 1, Fabrication Area : Employees operating a metal cutoff saw (machine 937) to cut materials such as but not limited to aluminum were exposed to amputation hazards created by unguarded saw blades, occurred on or about 5/13/15. c) 9000 River Rd Delair, NJ- Building 1, Fabrication Area : Employees operating a metal cutoff saw (machine 117) to cut materials such as but not limited to aluminum were exposed to amputation hazards created by unguarded saw blades, occurred on or about 5/13/15. d) 9000 River Rd, Delair, NJ- Building 11, Fabrication Area: Employees operated a table saw (machine number 2063) to cut materials such as but not limited to plastic where guards were not present to prevent having any part of their body in the danger zone, occurred on or about 4/16/15. e) 9000 River Rd, Delair, NJ- Building 1, Fabrication Machine Shop: Employees operated a Do-All band saw to cut materials such as but not limited to metal where guards were not present to prevent having any part of their body in the danger zone, occurred on or about 4/16/15. Aluminum Shapes LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 315978676, citation number 1, item number 4 and was affirmed as a final order on 5/3/2012, with respect to a workplace located at 9000 River Road, Delair, New Jersey. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
