29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). a) On or about March 16, 2015, while operating the Tesker Thread Roll number six, in the volume rods department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the rotating threading heads due to the 14 inch by 6 inch opening in the fixed guard. b) On or about March 17, 2015, while operating the Tesker Thread Roll number one, in the volume rods department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the rotating threading heads due to the guards being removed. c) On or about March 18, 2015, while operating the Tesker Thread Roll number three, in the volume rods department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the rotating threading heads due to the 13 inch by 6 inch opening in the fixed guard. d) On or about March 10, 2015, while operating the Tesker Thread Roll number six, in the volume rods department located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the rotating threading heads due to the 14 inch by 6 inch opening in the fixed guard. e) On or about March 26, 2015, while operating the Spartan Super Saw, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the saw operator was exposed to the unguarded spinning cutting blade. f) On or about March 18, 2015, while operating the Manual Four Spindle Upright Tapper in the Dyson Forge Threading (DFT) Division located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded spinning tapping die. g) On or about March 18, 2015, while operating the MA-5 number four NU-Tao Auto Tapper, in the Dyson Forge Threading (DFT) Division located at 53 Freedom Rd., Painesville, Ohio 44077, the machine operator was exposed to the unguarded rotating thread cutter. The Dyson Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 904620, citation number 1, item number 3 and was affirmed as a final order on 9/3/2013 , with respect to a workplace located Freedom Rd., Painesville, Ohio 44077.
