Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns, electrical shock, and smoke inhalation from fires, electrical contact, and explosion due to the use of unapproved electrical equipment: In the following instances electrical equipment was not operated or maintained in a safe condition: a)Barn 4 - The reel extension cord contained a broken ground prong. b)Barn 4 - A light socket and ungrounded plug adapter was utilized as a power source. c)Barn 5 - The runway light switch contained a broken cover. d)Hatchery, Incubator Room A - The fan motor for the #24 green machine was not grounded, and the conductors were not provided with strain relief. e)Hatchery, Incubator Room A - The fan motor for the #23 green machine was not grounded. f)Hatchery, Incubator Room A - The fan motor for the #19 green machine was not provided with strain relief. g)Hatchery, Incubator Room A - The fan motor for the #17 green machine was not provided with strain relief. h)Hatchery, Incubator Room A - The fan motor for the #16 green machine was not provided with strain relief. i)Hatchery, Incubator Room A - The fan motor for the #7 green machine was not grounded, and the conductors were not provided with strain relief. j)Hatchery, Incubator Room A - The fan motor for the #9 green machine was not grounded. k)Hatchery, Incubator Room B - The main light switch mounted on the east wall was not grounded. l)Hatchery, B Hatcher - The 240 volt conductors for machine #4 were not insulated. m)Between Barn 48 and 49, Generator Room - The unused opening in the transfer switch electrical box was not covered. n)Barn 42 - The #1 fan control switch was not grounded. o)Barn 42 - The #2 fan control switch was not grounded. p)Barn 42 - The #3 fan control switch was not grounded. q)Barn 42 - The #4 fan control switch was not grounded. r)Barn 42 - The #5 fan control switch was not grounded. s)Barn 42 - The #7 fan control switch was not grounded. t)Barn 42 - The #8 fan control switch was not grounded. u)Barn 42 - The conduit supplying power to the #4 fan was missing a knockout. v)Poland, Barn P1, Packing Room - The feed timer was not provided with a cover or insert. w)Poland, Barn P2, Packing Room - The #11 and #13 circuit breakers were connected together with a nail. A two-pole breaker is designed for this application.x)Poland, Barn P2, Packing Room - The outlet on the north wall contained a broken cover. y)Montville, South House - The #1 timer on the second floor was missing a knockout. z)Montville, North House - The #1 circuit breaker panel on the second floor was missing a cover. aa)Montville, North House - The conduit on the second floor stairway was missing a cover. bb)Unity, Maintenance Building - The four feed timers on the north wall were not provided with covers or inserts. cc)Unity, Barn Hallway - The outlet mounted under the Flock-Shock was not grounded. dd)Meadow Brook, Front Hallway - The feed timer on the west wall was not provided with a cover or insert. ee)Monroe, Barn 944 - The first floor light timer was not grounded. ff)Monroe, Barn 944 - The third floor light timer was not grounded. gg)Monroe, Barn 945 - The electrical box on the southeast wall was not grounded. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install and utilize approved gaskets, covers, hangers, conductors, switches, and fittings. Your company was previously cited for violations of this Occupational Safety and Health Standard which was contained in OSHA Inspection Number 300304987, Citation Number 2, Item Number 2, issued on 11/20/00 and OSHA Inspection Number 300298718, Citation Number 2 Item Number 2, issued on 5/28/99.
