29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 3 00), and/or Summary of Work-Related Injuries and Illnesses, (OSHA Form 300-A), a nd/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: The employer does not ensure that an equiva lent OSHA 300 injury and illness Log is maintained. This violation occurred on o r about October 13, 2010 at Lion Copolymer, LLC Baton Rouge, LA where the employ er failed to utilize a form equivalent to the OSHA Form 300 for the year 2008. P ursuant to 29 CFR 1903.19, within ten (10) calendar days of the the abatement date, the employer must submit documentation showing that it is in compliance wi th the standard, including describing the steps that it is taking to ensure that an injury and illness form equivalent to the OSHA Form 300 is being utilized.
